Panel Style (Drop Down Selection of the following Choices; P1, P2, P3, P4, P5)
Outside Frame Stye (Drop Down Selection of the following Choices; S1, S2, S3, S5, S6, S8, S9, S10, S12, S13)

Color Choice:
Color Choice Company : (Drop Down Selection of the following Choices; Sherwin Williams, Benjamin Moore)
Color Choice Color Name : (Text: Example: White Dove)
Color Choice Company Code: (Text?: Example: OC-17)
Desired Sheen: (Drop Down Selection of the following choices 15,20,30, 60)

Door and Drawer Pieces:
Piece Number: (Needs to be a 3 digit field)
Width: (Size in fractional Inches ie. 12 1/2 but have two scrolling options inches 0-100 and fractions 1/16 - 15/16)
Height: (Size in fractional Inches ie. 12 1/2 but have two scrolling options inches 0-100 and fractions 1/16 - 15/16))
Door or Drawer: (Radio button selection)

Need to have 50 spaces and then "+Add another piece" to add another Door and Drawer Piece

This form needs to be submitted then all the information needs to go to an excel or google sheets

I would love to put in a web hook in the submission button - I have give that information to you

do you want two fields here 1 number field for inches and other scroll down field for a fraction???
