Convert 8 page MS Word Document into fillable tables and form

I have an 8 page questionnaire form in MS Word format. There are simple questions that either require a text box or a radio button (yes/no). No drop downs, nothing advanced. I **want it to look professional, like the screen shot that I have attached from a different form that I already have.** I have also attached a screen shot from one of the pages from my form so you know exactly what type of questions I am asking. It is grouped into different categories so it will be easy to group when creating the new tables. Form must be password protected so I can edit, but I have ability to set password so user can input text into fields, but cannot edit any of the other text.
