Convert word questionnaire to on-line application

My understanding of project requirement: - You need to convert the questions in ppt to a web page (HTML page), where if a user submits the score will be calculated automatically - This project is not about developing an website; instead the HTML pages will be used in a application If my understanding is correct I would request you to please contact me. As per the mentioned requirement, the attached PPTX document needs to be converted into a webpage. I will deliver an HTML page which will be properly formatted using CSS. The score calculations will be done using javascript. The page can be served as static page. I will deliver a complete and properly formatted HTML page which can be easily tested in the browser. You can upload this page to your existing server and run the survey. Let me know if there are any changes to the requirements or any other deliverables are required.
