I have been doing Medical form filling for a long time now.... so this project is similar to that but you only need the specific fields. i am good at typing with 44wpm minimum and has been working in a call center for the last 5 years, so am very good in understanding english. I can get started right away. However i need to ask you this, i dont see the example document that you said you attached, it would have given me a better idea of what you require, also what you are planning on paying per record.
