Build Landing Pages and Enquiry form

For our recently launched site, we have a requirement for a number of jobs to finish things off.
Here’s a basic outline of what needs doing, all jobs can be quoted separately.
So, here’s the score. I’ve just started here at [url removed, login to view] and we’ve literally just launched our new website. So we are a relatively new, growing company who do live-in care for elderly people – essentially an option for people who were going to move into a care home, they can get someone to live with them and look after them, but they can stay in their own house.
There are many changes I’d like to make, but our budget necessitates that we just need to concentrate on the essentials for now. The site is based on the Concrete5 CMS ([url removed, login to view] – if you’ve not heard of it, it’s PHP based and seems pretty easy to work with)
So the first task is to...
Build an enquiry page based on the job application page template [url removed, login to view] , using the enquiry html form (ie the html in the contact us page, which is linked to our [url removed, login to view] setup). Would look something like the attached. This is an enquiry form for potential customers and would be located at [url removed, login to view]
There are other jobs related to this, and many more, but if we can find a designer/developer who can do the above, we can look at the other jobs later.
