Basic web lookup via a csv file

I need a web based lookup form. where a user enters in the following:
last 4 of the SSN:
First Letter of First Name:
Last Name:
Billing Date:
Amount Due:
it then returns all records in the CSV file with that information:
with the following Items Prefilled in a form file format:

Also if you need to convert the csv file to a db file than you must write the code to do that and have a admin section to administer that.
the database should be overwritten when executed.
NO PHP, NO MYSQL. This will run on a IIS server.
