Create an online form where the customer and staff can fill out and sign using an ipad or smart phone. The form is like a medical record which the customer and staff can open weekly to fill out again and sign. The form is stored on a server folder where the administration or user must log in using a password and email to access the branch folder. The owner can create new branch folder and staff to access that branch. Once the form is signed or dated, it cannot be edited by customers. Customer and staff can only add more info to form but not edit . Only the owner which is myself can edit all previous data input. Staff and customer can edit some data input such as prices but both signature is required to change. Form consists of tick boxes, drop down boxes, date boxes, text boxes and signature boxes. Date is automatically set next to all signature boxes when both parties sign. I have 4 different type of forms where the staff will choose before creating a customer record.
