Create a Simple 1D Image / eBook Cover - sleep apnea

I am looking for someone to create an, **Extremely Professional** looking, 1D (one dimensional) report cover. Please read all of the following and let me know if you totally understand. This is a VERY simple job and although there is only one cover at this time there will be more in the future. I do these in about 5 minutes each so DO NOT BID more than $3.00 each cover ..
