298887 HOSTING ORDER FORM

We need a order form just like the URL below, we need the different pages and the order progress. The form should be identical in design and fuction.

The form should have a admin area where the different plans can be set up to show on the order form.

The form should be set up to allow payment via Paypal subscription and via [url removed, login to view] recurring payment.

The order form should send the customer information to a database and a email to a email address set up in the admin area

The customer should be taken back to the site to a page set up in the admin

The form should allow a header and footer to be set up or changed in the admin.

The form should be able to be called via a secure connection (https).

We need full right to this form/script we will be providing this form to our clients
