BMI calculator HTML code for a website

I need a functioning BMI (Body Mass Index) Calculator to be posted on the home page of a website. The calculator should have/use/do the following: Standard SI formula, BMI = weight in kg/height in meters squared. Convert height from feet and inches to meters and weight in pounds to kg. Conversion formulas can be provided if needed. Display the results on the same page with a qualifier dependent on the following ranges: Underweight: < 18.5 Healthy: 18.5-24.9 Overweight: 25-29.99 Obese: > 30 The title "BMI Caculator" and the subtitle "Use this tool to determine if you have a healthy weight." A disclaimer "BMI is not a relevant measure for athletes or the elderly as it will over or underestimate your actual BMI." Color, borders, entry fields, and "Calculate" button. I would like bids on using a pre-designed graphic with fields and images in a jpg format and bids including graphic design. I may need other calculators and tools for this site and would love to develop a good working relationship with a coder.
