Create Fillable Form in PDF

Create a 24 page fillable PDF form with a button to send the form details to an email address and another button to Print the form along with the filled in details.

The PDF form would be a part of a website.

When the user clicks on the form link he would get the PDF form that he fills up and clicks Send to Email to send the form data. The PDF form also have a print button that allows the user to print out the form before sending.

The form data should be sent as an email attachment (xml) that can be imported into a database program. Adobe Designer does exactly what we need.

Your work would not involve working with the database but just creating the form and getting it to send the data and to get the print function to work.
