Customization work for Joomla Joombri Freelancer component - repost

I have a site connecting cash paying clients to professionals in the medical community. The site registers Patients, Doctors & Medical Facilities. I need custom work done that when a Doctor or Medical Facility registers to the site that during the registration process when they input their last name, address, city, state or zip fields the Doctor's results will return a list and they will be able to take control of the business details (if already opt'd in) or add a Doctor to facility or add a new facility with this Doctor or they can add a new doctor.
