Add file upload script to website

Hello friends,

I would like to add the "File Chucker" upload script to my website. This script does not need to truly integrate with my Magento installation (no login sessions or anything like that) -- I only want it to display within one of the pages so that it really "looks" like it's integrated well.

I will provide FTP information so you can install it to the website, then make it display in a Magento frame. I can customize it from that point.

If you prefer to use a different upload script, that is fine. The reason I choose File Chucker is because it has 1.) a progress bar, and 2.) the option for including additional fields such as "Name, Address, Phone, etc. Those are very important features I must have.
