Convert html form to magento cms page code incl css

We have a form in HTML, made by [login to view URL], this is html code with css and a js file. We need you to convert this form to new CODE so we can add it in a magento installation as CMS page, so that on the frontend it will be integrated in the page, also when browsed to directly; [login to view URL] The page should have a SEND button and by pressing that it should EMAIL us the form in a html email with nice styling, just as online. The form does NOT have to be saved to a database, ONLY send through email. After client clicks submit he should get a standard magento confirmation message that the form is send correctly. The form html code is attached for you to review. This form should be converted to php, but in a way magento cms pages can use it. Also the CSS should be updated so it looks nice in our magento shop. You should deliver a new code that we can copy/paste into the magento admin --> cms --> pages. If you need help files, they should be attached as well, we upload this to our server in specific folder. For this project it is not needed to get magento access to our shop, so we do not give that due to previous issues with people working in our backend. You should test your work in your OWN magento test environment and show us the working page. I think for experienced workers this is max 1 hour work, but please bid how many hours you need and what the hourly rate is and let me know. Or send a PM with a fixed price!
