Every day, our company communicates updates to policies and procedures that need to be shared with frontline staff. We require an application that will allow users to validate their understanding via a daily quiz. Questions will be created by our supervisory staff and uploaded to the system. Questions will require either a multiple choice, true or false, plain text (single line with a maximum text size) or open ended (multiple choice) answers.?
The application will run in a web browser and be written in PHP and MySQL. It should use an AJAX to minimize the need for page reloads and enhance the user experience. The application should have a large focus on the visual style and be visually pleasing. The application should operate across common browsers and not require any ActiveX or similar controls. The application should be light weight with fast load times. It should be optimized for speed.
A developer in the Philippines would be preferred but is not required.

When a quiz is set up, the test as a whole will optionally provide real time feedback on the results to the end user or instead send the results to the supervisor first for review before they are shared with the user. Any questions answered incorrectly by a user will result in a “coaching task?? being assigned to the supervisor. A coaching task is a task item that will appear in a task list available in the application when a supervisor logs on. This will track the date the test was taken, the date/time the coaching occurred and the date the task was closed. Each task will have a button to mark as completed that will capture comments from the supervisor detailing the nature of the coaching.

Reporting will exist at the end user level detailing the number of tests assigned to date, the average score and a detailed breakdown of the actual scores. Supervisor level reporting by user and in aggregate form should be used. Line or Bar graphs of historical performance are required.

Login access should be managed with a username and password. It should operate similar to a webmail type system. Password resets and retrieving of forgotten usernames should be possible using an external, pre registered, email address. Administrative users should be able to reset passwords, create and disable users. New user access should be via a registration form accessible to an end user and requires a two stage approval from the immediate Supervisor and the next level Supervisor.

The application should work in multiple languages including English, Chinese, and Korean.

Users of this application will fall into three categories:

A “Supervisor?? and “User?? are related in the real work in a supervisor/direct report working relationship. A “Supervisor?? can have many “Users?? reporting to them. A “Supervisor?? also is a “User?? and has their own “Supervisor??

The application will maintain a hierarchy of user/supervisor relationships. This can be updated via either a data entry feature in the GUI or via an Excel or CSV file upload.
