Create A HTML Contact Form With Attachment Option

I need help creating a simple HTML contact form which does the following: 1) Allows user to input name, e-mail, account #, message text 2) Allows user to attach files (Mainly Word documents or PDFS and images but I need as many file types available as possible - I cannot have a user get a message that a file type is not allowed.) 3) After a user submits the form, they get taken to the thank you page (Already created) 4) Form info and attachments need to automatically be sent to our e-mail Most importantly, I need this form to work on a standard HTML page. Our server is Yahoo Small Business - we need the HTML to parse as PHP and will need help if this does not work. We are on a budget and we need someone who is available to implement the form on our HTML page and do test runs to make sure it's working. We would be happy if this form was easily customized. (We can add/remove fields) And we would appreciate something that doesn't display ads or go through a 3rd party server. Thanks
