Customize Wordpress application

I have a responsive customized Wordpress site. We have developed "Get an Instance Quote" functionality and implemented on [login to view URL] page. Now we need two customizations to "Get an Instance Quote" application: 1. Ability to select pages from admin where we want this widget to be appeared. Currently it is coded in footer. We want someone who can make this positioning dynamic, so admin can select pages where he wants this widget to be appeared 2. Ability to create form elements for single service page ([login to view URL]). Provide word counts in the text box of "Get an Instance Quote" and proceed next. This page is single service page - showing pricing based on word counts. This page also shows a list of questions like SELECT A SERVICE TYPE, NAME THE INSTITUTION etc. These all are coded in PHP file. We need to make this fully admin controllable - admin should have option to add/delete different field types and labels (similar like contact form7). Upload document is mandatory, so it will be untouched. The site is live and this application is working fine. So NO AMATEUR CODER. These customization are to enhance application's flexibility. We provide database and theme files - selected developer must setup a demo instance and develop everything. Once everything done, the same needs to be set up over live site. While working don't 1.
