Damduce eCommerce - repost

My current site is hosted at [login to view URL] I have purchased the theme (developer version with psd) at [login to view URL] I would like a module created that will let me have the "Which country would you like to call" textbox similar to that on rebtel.com. Essentially, I intend to offer the services that rebtel also offers, at least the basics for now. I would require the frontend and backend created. The backend should allow: 1. The activation/deactivation of the extension 2. The ability to upload the rate table from csv 3. The ability to CRUD each rate within the table The front end should support: 1. Similar functionality as seen on [login to view URL] home page. 2. A sidebar widget that allows the same functionality but which can fit on the left or right sidebar on other pages e.g order, products etc. 3. Autocomplete 4. Country detection to suggest currency 5. Similar design, but easily themeable...this can be available in the admin/backend
