Create user fill in forms for my webpages

I have a few different websites. I want a basic form where I can have users create their own pages when they input the data. I want to be able to approve/disapprove/ and modify the data in the forms. I prefer newly inputted forms be sent to me via email, where I can approve/disapprove/and modify the data. Name Field A description field A .jpg field A website field Captcha Validation I want the users name to be added to the end of the site. EXAMPLE: [login to view URL] The sites these will be for are [login to view URL], [login to view URL], and [login to view URL] If a name is already submitted, ther will be a simple error message that says that name is already taken.
