Build database for website, and a program referring to database

I need someone to create a web-based database containing a list of questions, the list of options for those questions (IE Yes or No), the follow-up question for the option if chosen, etc. (See attached example.) Additionally, I need a a program that can read this database, pick one of the initial questions at random, provide the possible options to the user, picks one of the possible responses for whichever option the user chooses, and records each question and answer into a separate database. I'm unsure of what programming language you'd want to use, but as long as it can all be uploaded onto a website, then it will be fine. ## Deliverables • A web-based database containing the sample questions, responses, follow-on questions, etc (sample questions and possible format in attached excel file. Feel free to change the format if it makes the program easier). • A program that can read the spreadsheet, randomly pick one of the initial questions, provides the possible options to the user, picks one of the possible responses for whichever option the user chooses, and records each question and answer into a separate database. • A web-based database to record the information received from that program. 1) All deliverables will be considered "work made for hire" under U.S. Copyright law. Employer will receive exclusive and complete copyrights to all work purchased.
