Find Emails & Faxes From Website -- 2

I need someone to find me a list of doctors listed on the following website:

[url removed, login to view]

You will need to find all the doctors listed in the USA, Canada, and any other country that is listed on his website.

The website will ask you to enter a zip code, then it will list all the doctors that are in the zip code. I have attached a list of USA and Canada zipcodes which you can use for this task.

Once you find the doctor information, you will need to enter all the data into an Excel file with the following fields:

First Name

Last Name

Clinic/Company Name

Address

City

State

Zip Code

Country

Phone Number (format xxx-xxx-xxxx)

Fax Number (format xxx-xxx-xxxx)

Website Address

Please try to find as much of the information for each doctor and enter all the information for that doctor.

If for some doctors it does not list their email or phone or fax number, then please go to their website and find that information from their website.

Once you get all the doctors for all the zip codes and locations, please sort the Excel file and erase any duplicate entries.

You will probably have several thousand entries once you are done.

I am willing to pay $50 for every 1,000 entries.

I will only count the entries that have a valid email address and valid fax number.

Any entries that you can not find their email or fax number will not be counted towards the final count.
