I have worked with SharePoint for over 10 years. I would make use of an angular wrapper library I built a while ago to handle the forms and list interactions. Probably the most complex piece will be the user dashboard, but even that should be pretty straight forward. The best way to do this would be a SharePoint Hosted app, in which case the delivery would be a .app file that can be deployed to a private app catalog. I would be willing to assist with installation and deployment if needed. My bid is lower then I would normally do this for as I am trying to build up my reputation on here.
