Annotated Sheet Transcription

1. The audio file has been already transcribed by the client using a software. 2. The agent has to read the transcribed sheet, understand the conversation between Doctor and Patient and then accordingly fill the annotated sheet. Following are the details of the project. 1. You will be given two Excel sheets for each patient chart with the same filenames. 2. One Excel sheet named "Speech Input" will contain the converted speech to text content of the conversation between the doctor and the patient. 3. The speech input will contain the Time Stamp, Identification of the speaker (doctor or patient), and the conversation. 4. The other Excel sheet named "Annotated Output" will be in SOAP (subjective, object, assessment, and plan) format. 5. The Annotated Output sheet will have multiple pages named differently as Subjective-Chief Complaint, Subjective-HPI, Subjective-PMH, Subjective-PSH, Subjective-FH, Subjective-Medications, Subjective-Allergies, Objective, Assessment and Plan. 6. Reading through the Speech Input, you have to deduce what goes into each section. 7. Once you deduce and go to the particular page of the Annotated Output, you have to type the particular condition or detail as instructed at the top of the page, second column you copy the timestamp where the speech has happened from the Speech Input, and the third column there will be a dropdown menu from which you have to select the appropriate medical condition. 8. Reading the above speech, in the Annotated Output Excel sheet, you go to Subjective-Chief Complaint problem and (dictated speech) (Click on dropdown menu and select) Complaint Timestamp Condition/Type of visit High blood pressure - 11.234 hypertension Headache - 11.234 headache Suffering from yesterday 11.234 new patient visit Then in the Subjective-Medications page of the Annotated Output Excel sheet Dictated meds Timestamp (Click on dropdown menu and select) Medication Dosage Frequency Tylenol No. 3 25 mg 11.578 Tylenol No. 3 25 mg twice a day (once in the morning and evening)
