Add a menu option to Firefox browser

I run a company that proof reads manuscripts online. I wish to add a menu item to the toolbar of my firefox browser. When the menu item is clicked (an icon needs to be created for this new menu item) a drop-down list is displayed with 6 options (Bad Spelling, Incorrect Grammar, Poor Punctuation, Repetition, Incorrect Formatting ). When an option from this drop-down list is selected details of the option selected, the URL of the users current web-page and the IP address of the user are to be passed to a destination URL ([login to view URL]) in a form where they can be read into a database. EG. The user is on wepage [login to view URL] when they click the new menu icon. The user selects 'Bad Spelling' from the drop-down menu When this is selected the user is redirected to [login to view URL] and the following information is passed (as a cookie?) '[login to view URL]'l, 'Bad Spelling', 'their IP address' I am only suggesting the information is passed as a cookie. An alternative solution would be considered. This small piece of coding is to allow our proof readers to quickly report bad writing simply by clicking a menu and selecting from a drop down list. We will know which proof reader has made the report from their IP address and know what document they were looking at by the URL of the page they were viewing.
