Budget: max 350. A few months ago I awarded a project to a freelancer over here (using another account, don't want that company name in Google serps so that's why I'm using this new account). Overal he did a good job but there are still some bugs that need to be fixed. I tried to contact him, but as I already released the milestone and found some bugs after that, the communication was slow and since I'm on a soft deadline I just have to move on. Contact me for the URL that needs fixing. Beside some bugfixing, I need you to create some different form field elements like dropdown, multiple choice. But not normal form elements, some html/js/css skills will be needed to make them look good. (ul->li's instead of radiobutton elements for example). Will provide more details on chat ;)
