Need Help Finding and Displaying an Existing Form in Joomla Admin Panel

This is a simple project if you know Joomla very well. We will give 5 out of 5 feedback if done correctly and quickly. We had a custom user registration form created that displays on the site when we check a box next to each Article or Gallery album. This enables a "Login Required" option so users must register or login on the site first in order to view an Article or Gallery album they click. Attached are screenshots of the form we had coded and related parts of the feature. Again, this form still exists but is not displaying, so we need someone who understands Joomla very well that can find the form in the admin panel and make sure it displays when this "Login Required" checkbox is checked for each Article or Gallery album. Please see the attached screenshots for all the info you will need to complete the project successfully. WHAT WE DO KNOW: This may or may not help you find the form faster in admin panel. The form is already integrated with MailChimp API ([login to view URL]) and Textmarks API ([login to view URL]) for sending mobile numbers collected from the form to our client's TextMarks account and the email addressed collected thru the form to our client's MailChimp account.
