HI I want to have an interactive form for quick ordering from the phone, therefore we will have lets say COMBO1 <DROPDOWN VALUE 0,1,2,3> COMBO 2 <DROPDOWN VALUE 0,1,2,3> TEXT BOX SUBMIT BUTTON once the submit button is sent I want the values that were selected to be send to a url, the user to be redirected url?params=1_COMBO1_2_COMBO2_TEXT TEXT TXT something similar to that the project is do to a sample of those 2 text fields and send us the in design field and instructions on how to do it to replacate it on a large form we have
