Registration form for a car show

I need a relatively small web based registration form that will save the form info to a database. I also need to be able to pull this info up in web based reports. The other key factor here is that the form must be able to validate each field for errors and I also have to be able to validate (NOT PROCESS) credit card numbers. This form is going to be for a medium sized car show/ racing event in Colorado. I do not anticipate this form having more than 400 or 500 records, and it needs to have the following information: Section 1: First Name Last Name Email City State Year (dropdown box) Make (dropdown box) Model (dropdown box) Color (limited to 20 charadcters) Section 2: Are you participating in the car show? YES/ NO If yes, how many cars are you showing? (dropdown box 1-5 with hidden values to assign costs) Are you participating in the autocross? YES/ NO Year of race car (dropdown) Make and Model (textbox) Class of race car (dropdown box) How many sessions of the race will you be racing? (dropdown box with hidden value to assign costs) Do you have a work preference? (dropdown box) Are you participating in the slow drags? YES/NO (yes equals assigning a cost) Year of car (dropdown) Make and Model of car (text box) Are you participating in the R/C car autocross? YES/NO How many cars will you be racing? (dropdown 1-3 and each number assigns hidden value that equates to cost) Section 3: Would you like an extra t-shirt? YES/ NO Please choose size of your shirt (dropdown with sizes, and hidden value to assign cost) Please choose size of your 2nd shirt (dropdown with sizes, and hidden value to assign cost) Please choose size of your 3rd shirt (dropdown with sizes, and hidden value to assign cost) Would you like to rent a 10x10 canopy for $50? YES/NO (yes assigns cost) Would you like to rent a set of chairs for $25? YES/NO (yes assigns cost) Would you like to rent a generator for $65?
