234566 Simple Form Maker

I write contracts for a living and I will like to make my life a little easier if I can have a tool created that will simplefy the process. Basicly I do every thing off of templates were I will either fill in the missing information and then copy and paste allready made clauses ( little paragphs into the document) So basicly all I want is to have every thing upload into a program or system and all I have to do is enter things like my name telephone and then just pick or check a check box for witch of the 35 clause I want and thats what would appear on the contract the one I dont choose will say Intentionally Ommitted when the contract is printed. There will also have to be a place were I will cut and paste speacial specfications. And in some of the clauses I will require me to fill in some additional info if selected. This will have to be VERY user frendly
