I want a custom designed form created, but the form needs to be synced with Gravity Forms for collecting data. The form has some animation (see uploaded video) but the animation is optional. To bid on this project, you must know something about Gravity Forms API. The form will be embedded into a Wordpress page and the data from the form will be collected in the Dashboard in the Gravity Forms plugin. I've attached a PDF of the form and a video showing how the form should be animated as the user enters his/her data and what happens after the data is submitted. Important: Ideal if form can use bootstrap framework. Here is my version using bootstrap for the CSS: [login to view URL]

I have worked on gravity forms recently(Added a new field type through a custom plugin). I can work on this too. This project will be started right away. For the animation will be use CSS animation and bit of jquery. A little bit about me: I am the team lead, project manager and in most cases senior developer of a small team called CodeExecuter.
