I need a custom payment option on WooCommerce called "Pay By Phone".

• Collect the users telephone number and call back time

• Add data to order in WC backend

• Send data in admin email notifying of the new call back when the order is placed

We are running WP multisite and WPML, please ensure the code works on this environment.

I have attached a mockup of the final design. I will help with the style.
