Custom modifications to Enfold Theme

Dear Coders, I require a few custom modifications to my enfold theme here: [login to view URL] 1. Get rid of the [widgets_on_pages] text in the menu - I installed and uninstalled a plugin and for some reason this appeared. 2. Amend the right of the header to include the elements shown on the attached image. The Request Appointment button should be the same red as in their logo. These items will be hyperlinked to the respective pages and it must be responsive on all screen sizes and browsers. 3. Adjust the lower section with the hours and map to the same styling as the attached image. 4. Create a modal upon click to various buttons (Request Appointment) which includes a contact form as per the attached image (requires styling). You'll need to explain how I add this functionality to other buttons on the site as well. Upon click, the background will have a black overlay with 50% opacity and the Request Appointment form will be shown in the centre of the screen. Price Guide: under $50. Bids above rejected.
