Automated 'Members Form' on Revolution Slider

The website is a ‘Business Directory’ of company’s that include restaurants, bars, shops, rent-a-car, yacht charter etc. all in one destination. The website uses ‘WP-Lister’ Plugin and we have all the ‘add ons’ including custom field creation. We can add fields and drop down selection menus and customise the plugin menus as we choose. There are 122 Businesses on the Slider. There are 3 businesses per Slider. Approx. 40 Slides. Each Slide has 3 transparent pngs (1 on each layer) that covers ‘each business’. If a business owner purchases a ‘listing’ then their business becomes ‘activated’ on the slider ‘and’ they have their own page on the website. When their business is activated 'on hover' over their business on the slider a 'form' will appear with information taken from their 'add listing' page. As the member updates their page - their 'listing' page is automatically updated and their 'hover' form also updates. Every part of this site is complete apart from the automated 'Form Creation' that needs to appear automatically when they purchase a membership for their business. We need someone who can hand code the automated form, identify a solution to link the form to the 'add listing' page so that it only displays when a membership has been purchased. For further info please ask me. This is an urgent task and 'easy' for someone who knows action script and can work with revolution slider and wordpress.
