Code for Contact Form that Saves the Information on a Google Sheet

We need the HTML or LIQUID code that opens up a FORM as a Snack Bar that captures the client information on a Google Sheet and then follows a link. The FORM has to be able to lookup information in a Delivery Cost MATRIX (City, Cost, PurchaseLink), since the delivery cost and purchase link is different for each City. So when a City is selected by the client, the FORM displays the Cost and will follow the purchase link that corresponds when submitted. 3 fields of the FORM must be hidden initially, displayed only if: Type(field) = "Company" --> [See attached PDF] So we need: 1.- Code for the Subscribe Button: On click opens FORM as snack bar (not in another window) 2.- Code for FORM: When submitted saves information to Google Sheet and continues to link depending on City selected by customer 3.- File for Delivery Cost MATRIX example in any modifiable file type compatible with the FORM and can be uploaded to a server See attached PDF file with Button and FORM design. Also attached an example of the Delivery Cost MATRIX. The FORM must save the information on the same Google Sheet each time a client submits, so all the customers info is on the same file.
