We need to create an assessment form for our wordpress website that uses conditional logic. We were thinking that this form could be built using Gravity Forms, Caldera Forms or Jot Forms, but are open to suggestions. Additional FAQ: - Questions will be given a score and a weight. - Combined scores will determine if a client is the right fit for our services. - After a potential client completes the form, a statement will appear that tells them if they are a good fit, poor fit, not a fit, etc. - The form fields and fit output need to be sent to us via email. Can be as a pdf or just field text. As well the form fields and fit assessment need to flow into Salesforce. I am attaching an example of what the finished form will look like and an snippet of the how the questions would be scored and weighted. Please note that there may still be minor changes to the finished form.

Hi, I think the task is better achieved with Gravity Forms plugin which has both Salesforce addon and PDF addon. I will configure the fields the way so that final statement will be generate on final page, email with pdf sent to required addresses and salesforce record added. I will need to create GF addon plugin to achieve this, I have worked on similar projects before. I'm a senior WP dev with hundres of plugins written and customized.
