Online staff attendance register

We are a small insurance company with few staff who all work on our online php admin system at our head office and in other locations where we have smaller branches. We would like all staff to complete an online attendance register indicating the days they worked per week, the times they were at their branch / location (starting and ending time per day), the days they took leave, sick leave, etc. From admin side we would then like to view a summary of staff attendance whenever we insert a date or date range, leave taken, etc. We would like to integrate this with our existing admin software or use a plug-in instead of purchasing separate software and having them do it on there. If you have done this before or you have a ready-made plug-in then please let us have a look at it and decide on a price. Please do not send me a message to "discuss the project" as it should be very clear what we need.
