I have read details. As far as I am getting you, you have a website and want to add some functions to it. You have given a form, and you want to create the online. You want to send this to the database and entries should be visible in the admin area of wordpress setup. However, I am not clear on the second paragraph. What are you trying to say in that? I can include the form to be downloadable on the website, however, when people fill in, they will submit you filled form in MS WORD Format? I am bit confused on this.. Please clarify. Can u please explain more about the last field Signature? I am assuming that there will be 2 roles in the system, Admin and End User. Is that correct? Can u please explain about the attached form upload functionality? Will there be open registration for anybody or only admin will create the customer? I am assuming that the admin can update the submitted forms and admin can also submit a new record, correct can we discuss about the design part?

Hello Sir/Ma'am, We can make a form created that takes down the eligibility and can converts to medwatch’s uniformed eligibility excel spreadsheet . That way when someone signs up online, it goes straight to our admin wherein I don’t have to do data entry all day.. and i will also work on Group Eligibility form, we will need this to be a downloadable/uploadable and or able to be edited/submitted, form on the website for the everyday consumer. These will be people that are not with a group, that we will place inside our iMD group or general public group. This way we can enroll individuals.
