We conduct research. We would like a document designed, preferably in Microsoft Word, with our questions (that only we can edit) and fields for companies to write their [login to view URL] companies would then complete this form and upload it to our website. At any time, we should be able to extract collated data from all forms submitted (just like in formsite) in an excel spreadsheet.

Hi, instead of creating Word form or similar, would you rather consider creating a web form? You could email a link to the form to companies and ask them to fill in information. That would allow you to create various reports of collected data stored in your website database (example: mysql database etc). Looking forward to hearing from you.
