This is a credit card-sized membership card. This needs look creative and innovative--not a boring design with just text printed on a card. You can google "membership card" and look for some more innovative designs.

This card will be used by dental offices to offer a special dental program to their patients. Participants get a personalized card. The design for this contest is a sample card (front and back) that will be printed as a demo and given to dental offices.

See the attached PDF for a rough layout. This is just the information in the order it should be displayed in a rough design layout. We need something that looks upscale and high quality that gives the impression of an exclusive, elite dental plan that only the best patients receive.
