Conduct telephone survey with area employers

This project will be to contact up to 1000 Pittsburgh area small/medium sized businesses to gauge their 2011 hiring needs. These calls will be conducted with either the head of HR for the company or the owner, basically the person in charge of the company's hiring, and the survey itself will last just a few minutes with each organization. The survey itself will be approximately 6 questions long so ideally I need someone with some knowledge of business practices, great English speaking skills, the ability to enter information into a spreadsheet and detail exactly what the survey taker is saying.
I will provide a list of companies, the script (survey), and a spreadsheet (likely Excel). All companies will be in the 412 or 724 area codes of greater Pittsburgh
A sample survey would be something like:
1) do you expect 2011 headcount to a) increase, b) stay the same, c) decrease
2) which job functions are you targeting to add headcount in 2011 (sales, marketing, r&d, operations, IT, supply chain)
3) do you expect this additional headcount in 2011 to be a) full time employees or b) part-time/contractor employees
4) Do you anticipate using a 3rd party staffing firm to address these needs? if not, which methods will be used to find these new employees (job board, employee referrals, online/newspaper ads, in-house recruiter)
5) if you'd like a copy of this published survey to be sent to you please provide an email address
