Adding extra fields to Paid Membership Pro Checkout

What we need done is for you to add extra fields into the Paid Membership Pro plugin that we have downloaded and installed. We contacted there support team and they told us that we need to download and install the Register Helper plugin. We have done this, but i do not want to do any coding. Theses are the fields that we need added.

(Required) First name

(Required) Last Name

(Required) Mailing Address

(Required) City

(Required) State

(Required) Postal Code

(Required) Date of Birth

(Required) Email address

(Optional) Email Secondary

(Optional) Phone Number (Home)

(Optional) Phone Number (Work)

(Required) Phone Number (Cell)

(Optional) Company

(Optional) Company Website

(Optional) Who recruited you?

(Optional) Why did you join?

This is the plugin that has been downloaded and installed.

We have installed the Register Helper plugin and the Register Helper example plugin.

I have not configured anything. that's what you will be doing.

So basically the only thing you will be doing in configuring the custom fields in..
