Build Cordova/Phonegap App UI for cross platform android/iphone

(Budget is in lower half of the specified limit so please bid reasonably)

I'm writing the requirements in detail so that they're as thorough as they can be..

i. Build a cross platform Apache Cordova App using visual studio IDE (mandatory) and relevant plugins as required.

ii. The requirement for this app is just to build the UI and no functionality/server communication etc is required. The aim is to get a beautiful looking UI done for the app, functionality will be added to the UI by myself and is not a part of this requirement.

iii. The app will have below mentioned screens which can be statically linked as per given flow (for eg. on click of submit button the app moves on to next screen without performing any functionality)

iv. The app should not have any platform dependent code and should work on android, iOS, Windows phone platform (as all cordova apps do) & adjust to both orientations.

v. When moving from one screen to another the app or the page should not reload and the function onDeviceReady() should not be called again and again.

We're open to any kind of color combination, the only requirement is that it should give a professional quality look. A total of 6 simple screens are to be made with header, footer but no complex UI.

Screen header: Every screen will have the header as shown in file "0_Header". The top left corner of every screen would be a back button icon which should appear on every page except two pages: Registration & Home page, this button should work as a normal back button to go back to the previous screen. For future purposes we should be able to change the functionality of this button or select the previous screen to which it takes by use of javascript or any other way that you mention.

Screen Menu: At the top right corner of every screen there should be a vertical dots or bars pop up menu(the dots can be either as shown at top left in screen 0_Menu or they can be shown as bars with "more" written under them as shown in at bottom right in 0_Menu2). On tapping on it a menu bar should open which will have some items. Tapping on the menu items will take user to a dummy blank screen.

Screen footer: Every screen except screen 1 & 2 will have a footer as shown in "05_Schedule" file.

Screen 1: Registration screen (as shown in attached file "01"). The input boxes should have placeholder text. Apart from the inputs shown in file it will also have (a) a password field with a "show" link/icon which displays the password till its pressed, (b) a dropdown field "Gender" with values Male/Female. The phone field should enter only numerics and show user numeric keyboard only. The forgot password link does nothing & takes user to screen 2.

Screen 2: OTP validation screen as shown in file "02_OTP". The header will be the same as every screen (currently not shown in attached file). On clicking on the empty underscore OTP area, which is an input field, a numeric keyboard should appear. On clicking verify screen "03_Home" should come up.

Screen 3, 4, 5, 6: As shown in files "03_Home", "04_Detail", "05_Schedule", "06_Instructions". In screen 5 the days & time should be selectable by user. In screen 6 the image shown should be a gif which should keep showing animation and clicking on "Go to WiFi settings" should open wifi settings.

The sub heading font size should be quite smaller than the heading text size which should not be more than the font size shown in some of the screenshots. There will be some text here & there that might need to be put up once first draft version is built.
