Convert html or javascript from Aweber (opt in ''web form'') to a working .swf file

Project:

Take a small bit of html or javascript from an autoresponder service called [url removed, login to view] and convert it into a working .swf file.

The html is <form> script for an opt in box. The javascript is the same opt in form converted from html to javascript by Aweber.

I would require that the .swf file created be tested before payment was made.
