Convert Excel Document into a web form

I would like the attached document converted into a web form. There are multiple pages in the attached document. Yellow indicats where the input fields go. All of the sheets after the first are filled in based on the inputs from the first page.
The user needs to be able to change the legal information (the paragraphs above the signature on all the pages except the first budget page). Whatever they input will be the same for all the pages.
When the user finishes filling out the form and clicks submit the form should generate PDFs for download of all the data.
