Hello,

I need a simple plugin for WordPress that creates a form on front end, validates the input with captcha, records the data into a database table and uploads 3 images to a separate folder inside uploads folder.

I have already started putting it together so I have the html for the form, some PHP for the plugin and the database table. Attached are some screenshots.

Please note that this has to be a custom made solution and not achieve this using other plugins e.g. contact form 7 or contact db etc. I am happy if you use a captcha plugin but nothing else.

Please don't bid silly prices, this will not take more than 2 hours for someone who knows PHP and WordPress.

If you are successful at this task, there will be prospects for more work i.e build admin side of the plugin to modify data, show the data on front end etc.
