I need to create a Cost-Per-Action survey script that will gather consumer information, and then be offered to received information on various products and services from advertisers. I will be using incentives to get consumers to complete the survey, which may take several minutes and may require agreeing to receive free information from my clients. I plan to have several dozen products and services in this survey. Some will be shown multiple times. Most likely there will be 10-12 choices per page, and the consumer must select at least 3 in order to move to the next page. I must be able to record the consumers information on each product or service that they have selected to receive information on. See [url removed, login to view] for an example. This is NOT my site, just an example. I need something that is simple to use and expandable and affordable.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

The information gathered needs to be specific for each consumer.

I.e., if Joe Smith clicks on Product A and Bill Jones clicks on Service A, I need to know which is which.

As far as the number of fields and pages, I would like something that I could condense of expand. At this point, I do not have a set number of clients. Also, some will come and go with time. However, using the given example from the website, I would say we could start with 4-6 offers per page, with approxiamately 20 pages, would be a fair estimation.

The contact page needs to embed the consumers information into it, perhaps with a database. (View the source for the URL I supplied)

As far as maintenance, I need suggestions. I want to be able to send this information to my paying clients with as little effort as possible, since I will probably hire someone else to manage it for me.

don't want the consumer to breeze thru it. I get paid based upon how many of the offers they choose to receive information about. So, the longer they stay and click, the more profitable for me. However, if they start, but do not complete the I still get paid for the offers that they clicked on, although they have forfeited the iPod.

Also, the program must require that they click on at least one offer per page, in order to advance to the next page.

As far as the information, I want the option of sending it to my clients, as well as them being able to access it on their own.
