Convert a Template to a Website

I need an information capture section placed in my website where I can capture information from clients who want to sell their homes. I then need this information automatically forwarded to myself.
The information capture section must have the following sections: Title, First name, surname, daytime number, evening number, mobile number, email address, property address, post code, value of property, Property type, amount on mortgage, number of bedrooms, reason for selling, comments, submit button.
The information capture section needs to be placed in the following sections of the website: Home, Who we are, and get and offer.
I have attached an example of what the information capture section needs to look like.
