I have attached a document that is a 2 page standardized form issued by a government agency. I would like for the form to be re-created in HTML/CSS exactly as is. This means that if I print the page from my browser/save it as a PDF from my browser, it should look exactly the same as the source document. Please place as many comments as possible in the code - I would like someone new to code to be able to understand why you did what you did. I have dozens of additional documents that need the same treatment, so if you are successful with this one, I would be happy to give you the first bid on every one that will follow.
