I have a state promulgated .pdf form that I need converted to html code so that it can be integrated into our web site. The final "html" finished "fillable" form must be exactly (word for word, line for line, space for space, font for font, etc.) like the .pdf form that we will supply to you - see attached. Keep in mind that this is a state form and therefore the finished html product that will appear in our web site and printed by us, must be an exact duplicate form or the state will reject the printed hard copy on submission by us. The html form that you will duplicate for us will appear in our web site and will be a fillable form so that our customers can send it to us upon their completion and we can evaluate the filled in form and then forwarded it on to the state. Please be sure and ask any and all applicable questions as we will only accept a final product that is exactly as the state form that is attached. Please note that the form consists of two (2) pages, we only need the first page to be converted to an html fillable document, the second can appear only as an image.
