Design a wall hung template for on site Construction Projects

I want to design a template that my company can use for on site construction projects to make site supervisors and project managers accountable for all aspects of a specific project. The project should be able to be filled out in excel to distribute to ownership and also a hard copy wall hung option so that it can be physically written on site so that the client can see whats going on.
The template should incorporate a section for:
1. All materials/ finishes/ equipment- so that we know when the delivery date is anticipated to be on site.
2. Safety Check list: PPE, First Aid.
3. List of subcontractors for that given project, names, phone numbers, email address
I can collaborate and help the process as a few iterations may be needed to get this right.
